Early identification: Documented assessment of needs for palliative care patients (primary care sector)

Patients should receive palliative care much earlier than they often do, with a multidisciplinary team and a smooth transition from hospital to home. This increases the patient’s ability to die in their preferred place, with the supports they need.

    2) Identify patients who were screened and identified to have palliative care needs (denominator)
    3) Collect data on whether a palliative care needs assessment was completed (numerator)
