This past April, 52 teams submitted their annual Surgical Quality Improvement
Plan (SQIP)
, highlighting key achievements over the past year.
Explore the 2025/26 year-end highlights! Ontario hospitals achieved
measurable improvements in patient outcomes and sustainability. Nearly all
teams advanced in at least one key area in 2025/26.
We also maintain a SQIP implementation directory, which can
be obtained by emailing
ONSQIN@OntarioHealth.ca.
The directory is designed to connect teams, share tools, and accelerate the
spread of successful initiatives. Together, these resources are aimed at
supporting continued progress and collaboration across Ontario’s surgical
quality improvement community.
Updated geriatric variables will be available in the
Surgical Audit Report, providing enhanced insight into the unique needs and
outcomes of older adults in surgical populations. These data will help teams
better identify risk, tailor interventions, and strengthen decision-making
across the perioperative journey.
Register for the DASH campaign learning session on Wednesday, July 29, 2026,

practical approaches to reducing delirium and improving consistency in care for
high-risk populations.
