Hospitals are reminded to continue participating in the submission of Never Events data to the Patient Safety program. Thank you to all hospitals that have contributed to date. The next submission deadline is December 31, 2026, covering Never Events that occurred between January 1, 2026, and June 30, 2026.

Your participation is essential for advancing quality improvement, informing the development of the Never Events Prevention Plan (placemats), and enriching our ongoing community of practice sessions.

All placemat resources for the 15 hospitals never events are now available on the Ontario Health website! Resources are also available on the Quality and Patient Safety Program Community of Practice.
