For those in the Bundled Care program, the purpose of this Community of Practice is to allow members to support each other to successfully implement bundled care and funding. Through this Community of Practice, members will develop relationships and learn and develop new practices.

Note: This Community of Practice is no longer supported by Ontario Health, but serves as a place for learning, sharing, and collaboration among members.

The Bundled Care Compendium is a key resource that includes relevant policies
and Q & As from the first two years of bundled care implementation and can be located here (copy and paste link): https://bit.ly/3MfQI1B
