As
more health care moves from hospital to the community, there has been an
increase in the amount of services and interactions of providers in the
patient's home. This has become overwhelming and confusing for patients and
their families. While the role of a care coordinator has attempted to manage
this, multiple organizations have implemented this role resulting in additional
fragmentation and duplication across the system. Having a single contact,
'lead’ care coordinator for a patient will help reduce this confusion and
create a more unified, seamless healthcare experience for the patient and care
team. This IDEAS team was working on the following; For
25 patients of Dr. Gonu’s
practice that require multiple care coordination services (at least 2), the
primary contact lead care coordinator/case manager will be clearly identified
by all participating care coordinators by Feb 15, 2019
