Although many of the patients at Sunnybrook Holland Centre are being discharged on post-operative Day 2, there is room for improvement in education for patients and families, optimizing discharge processes, and communication with patients regarding discharge.
The aim of this IDEA's project was by September 28th 2018, 80% of patients who undergo primary hip and knee replacement at Sunnybrook’s Holland Centre will be discharged home by post-operative Day 2.
