Timely, comprehensive and legible discharge summaries from the General Internal Medical specialists to primary care was not occurring on a consistent basis. Primary care engagement also recognized the strong support for d/c summaries to be delivered within 48 hours. Timely discharge summaries alerted primary care to engage in more quicker appointments and comprehensive follow through.
