Telehomecare (6 month program) empowers patients to self-manage their chronic illnesses. Among the hospitals or CCACs who deliver Telehomecare, there is a lack of standardization of the discharge process. As part of best practices, effective discharge is an important component of care. There is an opportunity for improvement to enhance discharge transition for CHF and COPD patients.
