The Patient Safety Team at Ontario Health will be hosting this session to share high-level findings from the patient safety section of the 2024/25 QIP and provide an update for the upcoming 2025/26 year.
This session will include a focus on long-term care with a presentation from Ana Paula Anjos, Director of Quality & Risk at Kensington Gardens. Highlights will be shared about Kensington Gardens' work in implementing key enhancements to their incident reporting framework to promote a culture of continuous learning and improvement.
