A reminder to hospitals to continue participating in the submission of Never Events data to the Patient Safety Program at Ontario Health. The next submission deadline is June 30, 2026, covering Never Events that occurred between July 1, 2025, to December 31, 2025.

Your participation is essential to advancing quality improvement, informing the development of the Never Events Prevention Plan (placemats), and enriching our ongoing community of practice sessions. All placemat resources will be available on the Ontario Health website. Access the pressure injury, surgical, and medication-related placemats!
