Ontario Health has been collecting information about never events through the Never Events Hospital Reporting Initiative. To ensure meaningful and accurate feedback, we have created a survey to collect your feedback about this initiative!
Ontario Health requests that ALL hospitals complete this survey, regardless of whether they currently submit information to Ontario Health. This anonymous survey is intended to be completed by the individual most accountable for never event reporting within your organization, typically, the person responsible for patient safety, risk management, or quality improvement. Your participation in this survey will help strengthen Ontario Health’s voluntary reporting initiative, drive learning, and advance quality improvement.
Please submit your survey response by January 10, 2026.

Hospitals are reminded that the upcoming reporting cycle for Never Event submissions is due to Ontario Health by December 31, 2025. This cycle includes events that occurred between January 1 and June 30, 2025.
