Our goal was to minimize the number of potentially avoidable ED transfers for Long Term Care residents. Research suggests that ED transfers put residents at risk for a number of negative outcomes in addition to increasing ED wait times and resulting in unnecessary costs to the system. The Perley and Rideau Veterans Health Centre sends 7/100 every month on average, and some could have been avoided.
