Les communautés de pratique vous intéressent?

 

Ces communautés de pratique sont dirigées par des membres de Santé Ontario et d’autres organismes de soins de santé pour collaborer sur des programmes, projets ou initiatives d’AQ particuliers. Certaines sont publiques (c’est-à-dire que tout le monde peut participer) tandis que d’autres sont privées.

Si vous souhaitez créer votre propre communauté de pratique, communiquez avec nous à l’adresse quorum@ontariohealth.ca.

The purpose of this community of practice is to support Regional Renal Programs and their Quality Improvement Teams who are implementing the Access to Kidney Transplantation and Living Donation (AKT) Strategy. CoP members will gain access to provincial resources and, in addition to meetings, will use this online space to support learning and collaboration across Regional Renal Programs. Feel free to post your questions, useful resources and stories/experiences so we can all learn from each other.

The Community Stroke Rehabilitation Community of Practice (CoP) is for Regional Stroke Network staff, Ontario Health regional staff, and clinical and administrative staff involved in implementing the new Model of Care for Community Stroke Rehabilitation across Ontario. Through this CoP members will increase their awareness of leading practices and gain access to resources, tools and strategies that can be used to improve the quality of care provided within existing CSR programs and in standing up new programs. Members will work together to identify solutions to common challenges, share lessons learned, and collaborate towards future sustainability of programs.

The CCKO Nursing CoP is expected to perform the following functions:

1. To foster collaboration and enhance communication between the four regional lead sites and their respective satellite partners across Ontario.

2. Facilitate the sharing of best practices, knowledge, and experiences for the purposes of advocacy, building social capital and stimulating new learning through knowledge exchange when caring for children with medical complexities and their families.

The Halton Gender Affirming Care Community of Practice is for primary care physicians, specialists and other health professionals looking to build and share their knowledge and skill about gender affirming care to improve access for patients in Halton Region.

CoP members will gain access to resources, peers, champions, coaching, support, and evidence based best practice guidelines for providing not only gender affirming care but also primary care to transgender patients.

CoP members will share resources, challenges, strategies, and problem solve together to improve access to care in the communities of Burlington, Oakville, Milton, and Halton Hills. CoP members will also encourage colleagues in their formal and informal networks to participate and further increase the number of physicians in Halton that can provide this care.

LTC+ is a virtual care program developed in partnership with long-term care homes in Toronto, acute care hospitals and community services.

LTC+ provides immediate access to virtual care support and clinical guidance for LTC clinicians and staff.

The Ontario Surgical Network Pediatric CoP brings together pediatric surgical teams from all specialties and hospital types from across the province to form a community dedicated to quality improvement in surgery for better pediatric patient outcomes. Members will come together to coordinate surgical QI initiatives, share resources, coordinate meetings, and discuss challenges and successes. Members will use this private space to compare data, resources and collaborate on projects.

The purpose of this community of practice (CoP) is to support primary care providers who are implementing initiatives related to COVID-19. This includes, but is not limited to monitoring, vaccination, testing and care of patients with post-COVID-19 syndrome. Members are able to share resources, learnings, tools and strategies, as well as access clinical pathways based on best evidence. The CoP strives to bring together supports for the community through use of connecting calls and webinars, in addition to this online space which serves as a repository for resources as well as a space to support learning, sharing, problem solving, and collaboration.

This one stop hub aims to help you navigate primary care quality improvement information more efficiently. By joining this group, you will also be kept apprised of upcoming webinars, resources, and information for quality improvement offered through Ontario Health and other reliable sources. This community of practice (CoP) is for clinical, quality improvement and risk management teams across all health sectors developing their Quality Improvement Plan (QIP) – specifically addressing the new patient safety question in the narrative section. This CoP will give members the opportunity to come together with peers to discuss improvement opportunities and share learnings from patient safety related incidents and never events to help improve the culture around patient safety incident reporting.

Members of this group will have:
- Dedicated space to collaborate and share resources, best-practices and lessons learned;
- Gain access to data collection and tools to learn from patient safety incidents and never events; 
- Access to supports and upcoming monthly webinar information;
- Q&A opportunities 

The CoP is supported by clinical leadership and quality improvement specialists at Ontario Health.

This community of practice is for inter-professional team members from hospitals and community mental health agencies across Ontario who are participating in the Quality Standards Implementation Institute being led by Ontario Shores Centre for Mental Health Sciences, a specialty mental health hospital located in Whitby, Ontario.

The Institute will provide enhanced leadership and customization to support organizations with the implementation of statements from the Quality Standards of Schizophrenia (both Hospital and Community), Major Depression, and Alcohol Use Disorder. The Institute will provide a bespoke model of support ranging from guidance to more in-depth support depending on the organization’s needs and Statement selected.

Members will use this space to share comments and questions, implementation strategy materials and lessons learned in implementing quality statements across different settings.
