Ontario Health would like to thank the hospitals that have participated in the Never Events Hospital Reporting Initiative to date!

We are now in the 3rd reporting period of this initiative. This reporting period will close December 31, 2025, and hospitals are encouraged to report information related to never events that occur between January 1, 2025 to June 30, 2025.

We are also excited to share that planning for our Fall learning calendar is underway:

• Stay tuned for more details about our webinar on October 30th from 12pm – 1pm, focusing on building a safety culture
• Learning opportunities related to the prevention of pressure injuries
