Hello Everyone,
The deadline for submitting your
organization's Quality Improvement Plan is fast approaching. QIPs are due by April
1st, 2024. Here is some information that can support your QIP development.
Essential documents for QIP planning can be
found on the
QIP Navigator homepage.
They are located under ‘Annual Guidance Documents’ or in the Resources tab
under ‘Information to guide QIP development’.
Progress Report:
- Reflect on the change ideas from last
year’s QIP workplan
- Use it to highlight your organization’s successes
and challenges with or without change idea implementation
- Reflections should be concise. Think of
this section as what you would tell a peer if they wanted advice on using this
change idea.
- Add a row to include any new change ideas
that were implemented after your QIP was submitted last year.
Narrative:
Quality improvement plan (QIP) narrative
questions are an opportunity for organizations to provide context for their
quality improvement work. The new sections in the Narrative are highlighted in
red in the above image. In each section, organizations are encouraged to share
details that they believe people in Ontario would like to know about. It should
be framed as ‘what would you want to highlight to the people and communities
that your organization serves.’ Details on what to include in each section can
be found in the
2024/25 QIP Narrative Questions document.
Workplan:
- The workplan is the forward-looking portion
of the QIP that identifies the indicators, quality improvement targets, and
specific actions (i.e., change ideas) that your organization is committing to
for the upcoming year.
- Use the Target setting guide for QIPs to reference
use of provincial average, regional average or benchmarks
- Page 14-16 in the Quality Improvement Plan Program 2024/25 Guidance Document outlines what to include
in each field of the workplan
