Creating a safe, transparent, and empowering health care
environment requires more than just policies – it requires culture change. This
session will explore how health care leaders and teams can shift from a
blame-based culture to one focused on learning, accountability, and continuous
improvement.
By the end of this session, participants will be able to:
- Define the core principles of just culture and explain how
they apply in health care settings, including the importance of engaging
patients and families in fostering a fair and transparent environment
- Distinguish between blame-based and learning-based
approaches to incident response
- Identify strategies to embed safety culture into routine
practices and leadership behaviours
- Apply practical tools to support transparent event
reporting, staff accountability, and staff engagement in learning and taking
meaningful action after an adverse event
- Understand the role of leadership in fostering psychological
safety and organizational learning
- Engage patients and families as active partners in creating
a culture of safety by incorporating their perspectives into incident reviews,
improvement planning, and communication strategies
The session will be moderated by Dr. David Kaplan, Vice
President, Quality, Ontario Health, and the speaker panel will include:

Practices Canada
- Ioana Popescu, Consultant and Former Executive Director,
Patients for Patient Safety Canada
- Melissa Sheldrick, Patient and Family Advisor, Institute for
Safe Medication Practices Canada; Patient Safety Advocate, Ontario Health
This webinar is designed for health care leaders, managers,
patient safety specialists, quality improvement professionals, risk managers,
and frontline clinical staff involved in shaping organizational culture,
responding to safety events, or leading quality and safety initiatives. It will
also be valuable for anyone interested in advancing a culture of safety and
accountability in health care organizations.

- Building a Culture of Safety: Advancing Just Culture in Healthcare” is a
self-approved group learning activity (Section 1) as defined by the Maintenance
of Certification Program of the Royal College of Physicians and Surgeons of
Canada.
Attendance at this program entitles certified Canadian
College of Health Leaders members (CHE/Fellow) to 0.5 Category II credits
toward their maintenance-of-certification requirement.
