The opportunity for this project was to support high-quality person-centered decision-making conversations in chronic kidney disease through the implementation of palliative care mentorship. Mentorship will support multi-disciplinary staff in facilitating fulsome Goals of Care (GOC) discussions.When clinicians have increased skill and comfort with facilitating GOC conversations, it is our hope that this patient cohort will have improved access to upstream palliative care supports to align with their evolving GOC.
This IDEAS team focused on the following project level aim;
By November 1, 2018, 50% of prevalent patients on dialysis at Orillia Soldiers' Memorial Hospital (OSMH) will have goals of care documented in their EMR compared to baseline of zero.
