This Cohort 15 IDEAS team focused on improving access to palliative specialists:
They identified that patients do not access the right care, by the right provider, at the right time; thus patients’ needs are not being met in a timely way.Their IDEAS project aim was that 80 % of patients who meet the criteria of the surprise question, will be triaged to the appropriate palliative care specialist service provider in the sub regions of Haldimand Norfolk and Niagara (hospice, community, home) by April 2019.
