On the call, Ontario Health’s Patient Safety Team will share high-level patient safety findings from the patient safety section of the 2025/26 QIP and will provide an update on the the upcoming 2026/27 QIP.
Creating a safe, transparent, and empowering healthcare environment requires more than policies; it also requires culture change. In this session, you will also hear from a hospital and long-term care organization on how their health care leaders and teams reduce harm while creating an environment focused on learning, accountability, and continuous improvement.
