Ontario Health has developed an Integrated Quality Framework to help guide and align QI with our strategic objectives, the essential functions of QI and the six domains of health care quality.
The Framework describes the steps in doing a QI project and offers tools and resources to help you along the way.
Step 1 - Planning for Change
Are you new to QI? Learn about the science behind QI, how to form a QI team, and how to manage change within your unit, department, or organization.
The QI process begins with identifying and understanding the problem you are trying to solve. Your QI team will partner with patients and providers to identify a problem, understand the current state of the process, and identify the root causes of the problem.
Step 2 - Implementing Change
Begin to design and test change ideas to address root causes identified in step 1. The Plan-Do-Study-Act (PDSA) cycle is a way to test and adjust change ideas.
Step 3 - Sustaining Change
Formally implement your change ideas into practice in your unit or department, and once implemented, you will spread the change to other units or departments.
