Ontario Health is providing tools and resources to primary care providers who want to remotely monitor patients who have mild to moderate COVID-19 symptoms in the community. Safe, effective care can be provided to patients who do not require immediate hospital attention but could be at risk of developing more serious symptoms without monitoring.
Access the COVID@Home resource toolkit where you can find information on clinical pathways, how to order oxygen saturation monitors from the Ministry of Health and quality improvement tips for getting started, as well as a complete list of current Remote Monitoring Programs in Ontario with contact information.
