This open group is a general forum for those who are involved in the development of Ontario Health Teams. Members are welcome to ask questions, contribute to discussions, share resources, and collectively solve problems that relate to strategic decisions about the eight building blocks.

This space will also provide a forum to share general announcements, calls for participation, updates, and tools which are applicable to all OHTs.

This collaborative is for members of Ontario Health Teams (OHTs) priority population working groups looking to learn and develop skills necessary to plan for and implement a population-health management approach for older adults or those with chronic diseases.

Members of the collaborative will gain access to a toolkit of resources including examples from OHTs (please note, only OHT examples internally vetted by the OHT and approved for sharing will be posted) and an online discussion space. Those leading this work (e.g., OHT implementation leads, working group leads etc.) will also gain access to virtual collaborative meetings which include facilitated discussions by coaches to share learnings and solve problems with other OHTs and experts. The online discussion space is open to members to share learnings and ask questions between meetings.

Members of the collaborative will gain access to a toolkit of resources including examples from OHTs (please note, only OHT examples internally vetted by the OHT and approved for sharing will be posted) and an online discussion space. Those leading this work (e.g., OHT implementation leads, working group leads etc.) will also gain access to virtual collaborative meetings which include facilitated discussions by coaches to share learnings and solve problems with other OHTs and experts. The Patient, Family and Caregiver (PFC) Engagement and Partnership Community of Practice is a forum for individuals to exchange leading best practices in PFC engagement and partnership, support local work within their OHTs and engage with experts with experience in supporting meaningful PFC engagement. Overall, the CoP aims to enable successful implementation of OHT Implementation Support Transfer Payment Agreement deliverables, including development and implementation of PFC Partnership and Engagement Strategies.

The CoP brings together individuals working with and/or in an OHT, including patient, family and caregiver partners and engagement leads, to highlight different OHT initiatives around patient, family, and caregiver engagement and the challenges and successes OHTs are experiencing around implementation. The CoP is guided by its members and their needs.

The CoP will:
- share leading practices, resources, tools, and strategies that support patient, family and caregiver engagement and partnership activities.
- provide a constructive, positive, and supportive environment for participants to work together to identify solutions to issues and barriers.
- promote collaboration, innovation, and connections across OHTs.
- provide a forum to collectively identify issues that need to be communicated to the Patient Engagement Secretariat of the Ministry of Health and provide a venue for the Secretariat to share their work with members.

The CoP also aims to support the development of strong relationships among participants working towards the common goal of implementation of their OHT’s patient, family and caregiver engagement and partnership strategy and the Patient, Family and Caregiver Declaration of Values for Ontario.

This community of practice (CoP) is for OHT communicators to support the planning and execution of communications and community engagement work. Members will share ideas, information, resources and lessons learned to create efficiencies across OHTs.

This Community of Practice (CoP) is for the initial 12 Ontario Health Teams (OHTs) working on the Integrated Clinical Pathway for Chronic Obstructive Pulmonary Disease (COPD). The CoP is for team members and their supporting organizations who have received financial support to identify, prioritize, implement, and measure the impact of COPD change initiatives to improve outcomes for people living with COPD.​

The CoP brings together interprofessional teams including, but not limited to clinical leads, change management leads, project managers, leadership and front-line clinicians from primary care, home and community care, and acute care, as well as patients and care partners with lived experience. The CoP is meant to accelerate integrated, COPD care in Ontario and allow teams to build relationships, create efficiencies, learn and share in a collaborative way.​

This CoP is facilitated by the Ontario Health provincial ICP team, in collaboration with the OH Regions, and will be operationalized via this interactive online space that includes a discussion forum, document library of tools and resources, and a member directory for networking. The CoP also includes live touchpoints (i.e., interactive webinars) relevant to either the larger COPD community and/or subgroups within the CoP. ​

Note: The onboarding of additional CoP members beyond the initial demonstration programs will occur in FY 2024/25.​

This CoP is for OHT cQIP points of contact working on the successful development and implementation of your team’s cQIP. By participating in this group, members will be able to:

- Get their questions answered in a space dedicated to quality improvement in OHTs
- Access specific resources and supports to assist in your submission and progress reporting
- Get notified of any upcoming relevant cQIP events, webinars, or educational opportunities
- Share local best or leading practices, and adapt resources to advance your quality plan
- Identify emerging opportunities and address common barriers with cross-OHT collaboration
- Learn more about some of the indicators featured in this year’s cQIPs

This community of practice (CoP) is for OHT digital points of contact, other OHT members interested in digital health, Ontario Health Regional OHT Leads, Regional Digital Leads, and Regional Digital staff to:

- Facilitate sharing of local best or leading practices, thereby enabling spread and scale of successful digital/virtual initiatives through education
- Identify emerging opportunities/address common barriers
- Avoid unnecessary rework/duplication through sharing/awareness
- Realize potential efficiencies with cross-OHT collaboration and/or procurement
- Increase the digital/virtual maturity of OHTs across the province to enable them to better serve patients and meet their OHT objectives

The Community of Practice will be supported by a secretariat that will be staffed through Ontario Health (Population Health and Digital Excellence) and the Ministry of Health.

This community is for those working on evaluation and performance improvement in OHTs. The focus on the community will include performance improvement and evaluation plans as well as experience with implementing these plans. Members will gain access to a dedicated space to exchange ideas and participate in teleconferences and webinars. Members will also be able to share and adapt resources to advance their evaluation plans. The community is facilitated by the Health System Performance Network.

This community of practice is for the seven OHTs working on Leading Projects as part of the Home and Community Care Modernization Program in collaboration with Ontario Health regional contacts, Home and Community Care Support Regional Contacts, and Ontario Health provincial representatives. The intent of this CoP is to facilitate discussions and information sharing pertaining to forward-looking home care coordination and delivery transformation. Members will share information on latest guidance and best practices, discuss key topics of interest, identify and troubleshoot common issues and barriers, develop relationships, and celebrate successes. The CoP will consist of regular meeting touch points to discuss key priority topics as well as this collaborative space for ongoing sharing and learning across teams.

This CoP is for the OHT demonstration sites that have received seed funding and no loss provision to implement and operationalize the Integrated Heart Failure Care Initiative in collaboration with Ontario Health regional and provincial teams. The CoP brings together front-line clinicians, clinical leads, change management leads, and project managers across QBP funding hospitals, community care provider organizations and primary care etc., to ensure a collaborative approach to implementing this HF end-to-end care initiative. In addition, the CoP is now open all heart failure care providers and administrative staff (e.g., hospital and/or community health centres) and system partners (e.g., non-profit partner organizations) to build meaningful relationships and support peer-to-peer learning about the Integrated Heart Failure Care Initiative.

The CoP will be operationalized via this interactive online space as well as regular touch points focused on specific topics/content streams relevant to either the larger community and/or subgroups within the CoP.

This CoP is for all providers of LLP care and their system partners to work together and leverage the OHT model to deliver a full and coordinated continuum of care to improve outcomes and experiences for patients at risk for non-traumatic major lower-limb amputation.

This CoP brings together local and regional interprofessional teams including, but not limited to, clinical leads, change management leads, project managers, and front-line clinicians from primary care, home & community care, and acute care, as well as  patients and caregivers with lived experience, using a collaborative approach to accelerate integrated, LLP care in Ontario.

This CoP is facilitated by the Ontario Health provincial LLP team, in collaboration with the OH Regions, and will be operationalized via this interactive online space that includes a discussion forum, document library of tools and resources, and a member directory for networking.  The CoP also includes live touchpoints (i.e., interactive webinars) relevant to either the larger LLP community and/or subgroups within the CoP.

The OHT Planning Leads CoP is a self-organized forum for individuals with planning and operations roles in ‘approved’ and ‘in development’ OHTs. To join, members must have a defined planning, operations, or backbone role within one or more OHTs (e.g., OHT lead, OHT director, transformation lead, integration lead, planning & strategy led, project management backbone, etc). One member and/or delegate from each OHT can join this online space.

Members will informally exchange ideas and discuss key issues related to implementation of the Ministry of Health’s vision for Ontario Health Teams across the province.

We started in 2019 as a small group across several OHTs, and since then have continued to meet via videoconference to work through common challenges. This online space is intended to support our growing community between videoconference meetings by providing a shared space for resources and conversation.
