The Communications and Engagement Community of Practice is for the communication and engagement OHT staff to come together to exchange ideas, share best practices and support the planning and execution of communications and community engagement work. The Cop is guided by its members and their needs. Members will share ideas, information, resources and lessons learned to create efficiencies across OHTs.

The C&E CoP will:
- share successes and challenges, tools and strategies to support communications and engagement activities with our OHTs
- provide an open, informal, and supportive environment for participants to work together to identify solutions to issues and barriers
- promote collaboration, innovation, and connections across OHTs

The CoP also aims to support the development of strong relationships among participants working towards the common goal of implementation of their OHT’s communication and engagement strategy.

This Community of Practice (CoP) supports Ontario Health Teams (OHTs) actively engaged in the design, implementation, and improvement of Chronic Disease Prevention and Management (CDPM) and Integrated Clinical Pathways (ICPs) for Chronic Obstructive Pulmonary Disease (COPD), Heart Failure (HF), and Lower-Limb Preservation (LLP). This CoP brings together interprofessional teams including, but not limited to, clinical leads, change management leads, project managers, leadership, and front-line clinicians from primary care, home and community care, and acute care across OHTs, as well as support partners and subject matter experts to advance integrated care in Ontario.​

By participating in this group, members will be able to:​
- Get their questions answered in a space dedicated to CDPM and ICPs in OHTs​
- Get notified of any upcoming relevant events, webinars, or educational opportunities​
- Share local best or leading practices​
- Identify emerging opportunities and address common barriers with cross-OHT collaboration​

This CoP is facilitated by the Ontario Health (OH) provincial team, in collaboration with the OH Regions, and will be operationalized via this interactive online space that includes a discussion forum, a document library of tools and resources, and a member directory for networking. The CoP also includes live touchpoints (i.e., interactive webinars) relevant to the larger community and/or subgroups within the CoP.

This open group is a general forum for those who are involved in the development of Ontario Health Teams. Members are welcome to ask questions, contribute to discussions, share resources, and collectively solve problems that relate to strategic decisions about the eight building blocks.

This space will also provide a forum to share general announcements, calls for participation, updates, and tools which are applicable to all OHTs.

This community of practice is built on peer-to-peer learning and is for all people who are part of, or affiliated with, an OHT doing evaluation and improvement work in priority areas.

​It also brings together data experts, evaluation leads, project or program managers, quality improvement and implementation leads as well as clinical providers involved in improvement work.

The CoP will be facilitated through regular virtual meetings and this online group space.

CoP members will gain access to:
- Learning from peers and content experts
- Practical guidance on using data and evidence to inform decisions and improvement
- A space to share and discuss their work with peers
- Ways to draw evaluation and improvement approaches together, using evidence to understand what is working and why, as well as to test and embed changes that improve care
- Tools, resource and guidance on best practices

CoP members can contribute by sharing their learnings and resources, ask questions, building connections and working through challenges together. Over time, members help build a shared practice, strengthening how evaluation and improvement are carried out across OHTs.

The CoP is facilitated by Health System Performance Network with support from Ontario Health OHT Implementation and Strategy team members.

The OHT Planning Leads CoP is a self-organized forum for individuals with planning and operations roles in OHTs. To join, members must have a defined planning, operations, or backbone role within one or more OHTs (e.g., OHT lead, OHT director, transformation lead, integration lead, planning & strategy led, project management backbone, etc). One member and/or delegate from each OHT can join this online space.

Members will informally exchange ideas and discuss key issues related to implementation of the Ministry of Health’s vision for Ontario Health Teams across the province.

We started in 2019 as a small group across several OHTs, and since then have continued to meet via videoconference to work through common challenges. This online space is intended to support our growing community between videoconference meetings by providing a shared space for resources and conversation.

The Ontario Health Primary Care Network (PCN) Knowledge Exchange Hub (OH PCN Hub) is a collaborative space designed to bring PCN participants together to build relationships, share knowledge, and work collectively to focus on advancing PCNs.

The OH PCN Hub welcomes clinical and administrative participants who support the planning and delivery of PCN activities. Our goal is to complement OH regional work and support collaborative sharing and learning across Ontario. 

OH PCN Hub members will gain access to:
- PCN peers across the province and subject matter experts/champions 
- Tools and resources to help navigate PCN information more efficiently, including a PCN toolkit
- Webinars and information for PCNs offered through OH and other reliable sources

For the benefit of the provincial PCN community, members are encouraged to share in this space:
- Examples of primary care leadership and innovations
- Resources, tools, and templates
- Challenges, lessons learned, and advice 
- Upcoming webinars and other events that are open to a provincial audience

While OH does not endorse content shared in the hub, it provides a platform for knowledge exchange and collaboration across PCNs. This space will evolve as we learn together.

The OH PCN Hub is facilitated by the OH Primary Care Team.

We recognize and respect the role of regional partners and networks and do not intend to replace or compete with their local support. Participants should continue to contact their OH Regional Teams for questions and support related to program expectations.

The Patient, Family and Caregiver (PFC) Engagement and Partnership Community of Practice is a forum for individuals to exchange leading best practices in PFC engagement and partnership, support local work within their OHTs and engage with experts with experience in supporting meaningful PFC engagement. Overall, the CoP aims to enable successful implementation of OHT Implementation Support Transfer Payment Agreement deliverables, including development and implementation of PFC Partnership and Engagement Strategies.

The CoP brings together individuals working with and/or in an OHT, including patient, family and caregiver partners and engagement leads, to highlight different OHT initiatives around patient, family, and caregiver engagement and the challenges and successes OHTs are experiencing around implementation. The CoP is guided by its members and their needs.

The CoP will:
- share leading practices, resources, tools, and strategies that support patient, family and caregiver engagement and partnership activities.
- provide a constructive, positive, and supportive environment for participants to work together to identify solutions to issues and barriers.
- promote collaboration, innovation, and connections across OHTs.
- provide a forum to collectively identify issues that need to be communicated to the Patient Engagement Secretariat of the Ministry of Health and provide a venue for the Secretariat to share their work with members.

The CoP also aims to support the development of strong relationships among participants working towards the common goal of implementation of their OHT’s patient, family and caregiver engagement and partnership strategy and the Patient, Family and Caregiver Declaration of Values for Ontario.

This online discussion space is part of the Remote Care Monitoring (RCM) Quality Community of Practice (CoP), a province-wide network for teams supporting patients through remote care. The forum is a place to connect, learn, and collaborate.
The CoP brings together clinical teams, program leads, Ontario Health staff, and subject matter experts to improve the quality of remote care in Ontario. Through virtual meetings and this shared space, members can:

• Discuss key quality metrics and how to use them
• Share local experiences, best practices and resources
• Collaborate across sites
• Stay updated about new evidence to ensure alignment with evolving guidelines and standards

This RISE Primary Care Shared Space brings together those leading OHTs/PCNs and responsible for PC TPA deliverables (e.g. PC AA).

The purpose of this group is to:
• Share resources (peer learning and sharing recordings and materials, deep dive materials, briefs, rapid syntheses etc.)
• Encourage networking, sharing, collaboration and problem solving among group members
• Provide evidence and experience informed core concepts and principles on PC related topics connected to OHT/PCN deliverables and building high-performing primary care
• Continue to provide evidence-based supports for population-health management (a primary driver of high performing PC)

The group will be facilitated through this online group space and live touchpoints (peer sessions, deep dives etc.) hosted by RISE.

This shared space is a closed group for those in OHTs/PCNs leading primary care (PC) work (e.g., supported attachment, optimizing team-based care etc.) which are participating in RISE deep-dive sessions. ‘Deep-dives’ are interactive improvement sessions to apply learnings of a specific PC concept (e.g., developing a reliable attachment process) which help OHTs/PCNs to achieve their deliverables and build a high performing PC system. • Materials and recordings for deep-dive sessions
• A place for members to share templates and tools they’re testing
