RISE Primary Care Problem Solving Space

This shared space is a closed group for those in OHTs/PCNs leading primary care (PC) work (e.g., supported attachment, optimizing team-based care etc.) which are participating in RISE deep-dive sessions. ‘Deep-dives’ are interactive improvement sessions to apply learnings of a specific PC concept (e.g., developing a reliable attachment process) which help OHTs/PCNs to achieve their deliverables and build a high performing PC system. This group will be facilitated through regular virtual meetings and this online group space which will provide:
• Tools, resources, and guidance on best practices
• Materials and recordings for deep-dive sessions
• A place for members to share templates and tools they’re testing
• A space for members to network, contribute their learnings, and problem-solve together
