Les communautés de pratique vous intéressent?

 

Ces communautés de pratique sont dirigées par des membres de Santé Ontario et d’autres organismes de soins de santé pour collaborer sur des programmes, projets ou initiatives d’AQ particuliers. Certaines sont publiques (c’est-à-dire que tout le monde peut participer) tandis que d’autres sont privées.

The Ontario Health Oncology Patient Navigation Group is a private forum for healthcare providers working in Oncology Patient Navigation roles in Ontario. Members of this group support patients with cancer and their families throughout the cancer journey.

By joining this group, members will be able to:
• Collaborate with others through this virtual forum
• Share and discuss best practices, emerging issues, tools and resources
• Build their professional network

The Oncology Patient Navigation Group has two mandatory virtual meetings per year during which members share knowledge, learnings, and best practices in supporting patients through the cancer journey.

This is a private group for members of the Ontario Cancer Symptom Management Collaborative (OCSMC) committee at Ontario Health. The OCSMC supports the implementation, uptake and management of relevant Patient Reported Outcome Measures (PROMs) within regional cancer and partner sites. The OCSMC also supports high quality, consistent response and management of patients’ physical and emotional symptoms, both for in-person and virtual care models. Through multidisciplinary engagement, the OCSMC strives to promote evidence-based symptom management guides for patients and providers, as well as establish quality improvement initiatives to improve care outcomes.

This platform provides a space to:
• Collaborate: Troubleshoot common issues and barriers and celebrate successes
• Network and build relationships
• Share knowledge and resources (e.g., webinars, research, etc.)
• Access OCSMC materials such as slide decks and meeting recordings

This page is monitored by the Symptom Management team within Cancer Clinical Programs at Ontario Health, however it is intended to be a collaborative space for the OCSMC committee. Please reach out to us at OH-CCO_SymptomManagement@ontariohealth.ca.

The Ontario Complex Malignant Hematology Resource Hub is a place for networking and knowledge exchange to facilitate connection and resource sharing between Ontario Health (Cancer Care Ontario), CMH service provider sites, and other health care providers and community organizations.

Members of this group will gain access to administrative and clinical resources, upcoming Lunch & Share opportunities and past recordings, network with subject matter expertise, and more.

This Resource Hub is supported by the Ontario Health (Cancer Care Ontario) Complex Malignant Hematology team.

The Ontario Health Psychosocial Oncology Mental Health Community of Practice is for healthcare providers, researchers and administrators working in (or interested in) oncology and mental health. Members of this community of practice share information and expertise in order to advance the conversation about oncology and mental health and improve the patient experience.

By joining this community of practice, members will be able to:
- Collaborate with others through this virtual forum and regular meetings
- Share and discuss best practices, emerging issues, tools and resources
- Stay informed on issues relating to their area of practice
- Build their professional network

In addition to this virtual forum, this community of practice meets virtually three times a year, where speakers present on a variety of topics and members are invited to participate in discussions.

The purpose of the Psychosocial Oncology (PSO) Program Committee is to advance the mission and vision of Ontario Health (Cancer Care Ontario) as outlined in the relevant Ontario Cancer Plan by contributing to the development and implementation of the PSO Program's priorities. The work of this committee is intended to support the psychosocial needs of patients and families while promoting the values of person-centred care.

Membership
• Chair - Provincial Head, Psychosocial Oncology
• PSO Regional Leads from each Regional Cancer Program (14), appointed by the Regional Vice President
• Other representatives from the Regional Cancer Program nominated by regional leadership (e.g. Regional Director) or Regional Lead
• Patient/Family Advisors
• PSO, Patient Education and Survivorship Program manager, staff

This space will be used to share updates, resources, and learnings that will support the work of the committee.
