Les communautés de pratique vous intéressent?

 

Ces communautés de pratique sont dirigées par des membres de Santé Ontario et d’autres organismes de soins de santé pour collaborer sur des programmes, projets ou initiatives d’AQ particuliers. Certaines sont publiques (c’est-à-dire que tout le monde peut participer) tandis que d’autres sont privées.

For those in the Bundled Care program, the purpose of this Community of Practice is to allow members to support each other to successfully implement bundled care and funding. Through this Community of Practice, members will develop relationships and learn and develop new practices.

Note: This Community of Practice is no longer supported by Ontario Health, but serves as a place for learning, sharing, and collaboration among members.

This Central East Stroke Network (CESN) Community of Practice is a forum for leaders, educators and front-line clinicians that support implementing and sustaining quality stroke unit care (ISU, acute/rehabilitation units). The forum is a platform for members representing organizations within the Central East Stroke region (Simcoe, Muskoka, York, Durham & HKPR) to share experiences, foster collaboration, and support each other to advance best practice and standardization.

Through this CoP members will increase their awareness of best practices and provincial initiatives and gain access to resources, tools and strategies that can be used to improve the quality of care provided within existing stroke units (ISU, Acute/Rehab units) and in implementing new units.

The Community Stroke Rehabilitation Community of Practice (CoP) is for Regional Stroke Network staff, Ontario Health regional staff, and clinical and administrative staff involved in implementing the new Model of Care for Community Stroke Rehabilitation across Ontario. Through this CoP members will increase their awareness of leading practices and gain access to resources, tools and strategies that can be used to improve the quality of care provided within existing CSR programs and in standing up new programs. Members will work together to identify solutions to common challenges, share lessons learned, and collaborate towards future sustainability of programs.

The CCKO Nursing CoP is expected to perform the following functions:

1. To foster collaboration and enhance communication between the four regional lead sites and their respective satellite partners across Ontario.

The DASH community of practice (CoP) is for clinical teams working on the development and implementation of quality improvement measures to identify, prevent, manage and reduce hospital acquired delirium. It also brings together interprofessional teams who have an interest in reducing delirium rates at their site.

The Emergency Educators Network of Ontario (EENO) community of practice is for Emergency Clinical Nurse Educators working in staff development and implementation of policies/procedures/standards of care to improve clinical practice for patients in the Emergency Department setting.

This group brings opportunity to collaborate and knowledge share, co-ordinate professional development activities, and receive updates from provincial partners.

The CoP will be facilitated through regular virtual meetings and this online group space.

Within this online space, CoP members will gain access to:
• A document archive of all EENO Inquiries
• A forum that may be used for inquiries and knowledge sharing

CoP members will be able to network, contribute their learnings, and problem-solve together to improve nursing practice and process in Emergency Departments across Ontario. Members of the CoP are expected to share relevant policies, procedures, and resources in response to inquiries sent out by the members.

The Gender Affirming Care Community of Practice is for primary care physicians, specialists and other health professionals looking to build and share their knowledge and skill about gender affirming care to improve access for patients in Ontario. The CoP is facilitated by Prime Care Family Health Team.

CoP members will gain access to resources, peers, champions, support, and evidence based best practice guidelines for providing not only gender affirming care but also primary care to transgender patients.

CoP members will share resources, challenges, strategies, and problem solve together to improve access to care. CoP members will also encourage colleagues in their formal and informal networks to participate and further increase the number of physicians that can provide this care.

The NLOT & LTC+ Learning Community is dedicated to NLOT and LTC+ teams across the Toronto region. Our mission is to provide a platform for knowledge sharing, capacity building, and best practices while addressing common challenges. The NLOT Learning Community aims to foster an environment for peer mentorship, collaboration, and continuous learning, with the goal of enhancing care quality for residents in long-term care (LTC) settings.

The NLOT Learning Community will be facilitated through quarterly virtual and/or in-person meetings, as well as this online group space that members can subscribe to where they can log in to access:

This Quality Community of Practice (QCoP) is for health system partners who:
- Are responsible for quality programming on teams or in organizations
- Are interested in easy access to quality improvement (QI) projects and resources
- Have a need for extra support with Quality Standards and QIPs

The purpose of the QCoP is to support Ontario Health (OH) funded health service providers and northern partners by aligning the quality agenda across the north using the North Regions’ Integrated Quality Framework’s strategic objectives of embedding quality and driving system improvement.

The QCoP will provide a forum to advance the foundations for QI in support of the OH North Regions’ Operational Plan and Annual Business Plan targets. In addition, it serves as a conduit between OH Provincial Clinical Institutes and Quality Programs (CIQP) and OH northern leadership. The QCoP also brings together those who have passion for continuous QI and are interested in connecting with other QI leaders in the field.

The QCoP will be facilitated through bi-monthly virtual meetings and this online group space.

QCoP members will gain access to:
- Tools, resources, and guidance on best practices
- Implementation resources
- Subject matter experts in the field
- Ongoing data reports​
- Hands on QI support

The Central Region Palliative Care Collaborative Knowledge Centre is dedicated to supporting the implementation of Adult Community Model of Care through a cooperative and comprehensive platform. This Knowledge Centre brings together Clinical Coaches, OHT Leaders/Project Leads, Clinical Co-Leads and members of the Regional Implementation Team. The OH Toronto Palliative Care Collaborative is dedicated to supporting the implementation of the Adult Community Model of Care through a cooperative and resource-rich platform. The Oncology Patient Navigation Group is a private forum for healthcare providers working in Oncology Patient Navigation roles in Ontario. Members of this group support patients with cancer and their families throughout the cancer journey.

This is a private group for members of the Ontario Cancer Symptom Management Collaborative (OCSMC) committee at Ontario Health. The OCSMC supports the implementation, uptake and management of relevant Patient Reported Outcome Measures (PROMs) within regional cancer and partner sites. The OCSMC also supports high quality, consistent response and management of patients’ physical and emotional symptoms, both for in-person and virtual care models. Through multidisciplinary engagement, the OCSMC strives to promote evidence-based symptom management guides for patients and providers, as well as establish quality improvement initiatives to improve care outcomes.

This platform provides a space to:
• Collaborate: Troubleshoot common issues and barriers and celebrate successes
• Network and build relationships

This CoP will give members the opportunity to come together with peers to discuss improvement opportunities and share learnings from patient safety related incidents, hospital never events, quality improvement plans, and other work to help improve the culture around patient safety.

The goals of this CoP are to:
- Collaborate, share, and learn about best practices in patient safety;
- Highlight work being done within your team or organization;
- Access quality improvement tools and resources, including those related to the Never Events Hospital Reporting initiative; and
- Engage with peers in webinars, discussions, and Q&A.
