This Community of Practice (CoP) is for Long-Term Care (LTC) Falls Prevention Clinical Leads (RN, RPN, PSW, Restorative Care Worker, OT, PT) working on the development and implementation of a quality improvement project to reduce the rate of falls and fall-related injuries, and improve resident safety by sharing best practices, accessing resources, and building capacity.
It also brings together LTC Administrators and members from the North East Long-Term Care Network.
CoP members will gain access to:
• Resources: Tools, evidence-based strategies, subject matter experts, and training programs to enhance falls prevention efforts.
• Quality Improvement and Best Practices: Support for implementing targeted falls prevention strategies and leveraging local data to reduce fall rates and potentially avoidable ED visits related to falls in the North East region
• Collaboration and Shared Learning: Opportunities to network, exchange experiences, and share best practices in falls prevention. The initiative also engages residents, families, and caregivers through the Stay on Your Feet program.
• Capacity Building: Strengthening staff skills through education, mentorship, and peer learning, fostering a culture of safety with ongoing training and support.
The CoP will be facilitated through regular virtual touchpoints and this online group space on Quorum.
The CoP will be supported by Jennifer Osesky (Project Sponsor), Jane Mason (NE SSPDI LTC Lead), Maria Baldia-Torres (NE SSPDI Specialist), and Jon Brunetti (NE SSPDI Population Health Lead), the North East LTC Network, and Ontario Health North East System Strategy, Planning, Design and Implementation (Long-Term Care and Population Health) team.
