The focus of this session will be on the Coordination Between Dedicated SCD Centres and Local Health Care Teams.
In the session, we will discuss implementation of the sickle cell disease quality standard using coordinated approaches between dedicated SCD centres and local health care teams in pediatric and adult care settings across Ontario. A panel of experts will explore tools and strategies to improve communication and care coordination to support people with SCD.
