Patients are taking longer than they should to get to rehabilitation services from the onset of stroke; at start of project, early data showed average of 15.65 days from acute admission to transfer to rehab. Evidence based practice indicates that stroke survivors need early and intensive access to rehabilitation for the best outcomes and QBP targets are transfer in 5 - 7 days of stroke onset.
