Complex patients are known to be on many medications simultaneously; a preliminary review of SELHIN Health Link data reveals an average number of 15 medications (range 2-38). Obtaining a consistent and up-to-date medication list is known to be a challenge for complex patients with multiple touch points in the system. Lack of single point of data entry available to all providers is also an issue.
