Paul is leaving the hospital after 7 days in hospital for treatment of his depressive symptoms. He is not sure what the plans for life after hospitalization are.He is given a “shopping list” of people and programs that he could call and an appointment with a psychiatrist. Also included, is a list of various medications that he may or may not still be taking.Paul hasn’t been involved in much planning and was only told the morning of his discharge that he would be going home in a couple of hours. He is not sure if and to whom this discharge summary may have been given.He leaves the hospital confused and overwhelmed and may likely return.
The aim of this IDEAS project was by January 31, 2018, 50% of adults with a diagnosis of Major Depression who are transitioning from inpatient psychiatry at SAH to community mental health and addictions services within SAH and at CMHA, or vice versa, are in receipt of a clear, comprehensive transition plan that is shared with all appropriate service providers.
