The residents and families of HJ
McFarland Memorial Home have responded to the most recent patient/caregiver
survey and expressed there are improvement opportunities in the area of access
to external healthcare providers.
This finding is confirmed by staff
identifying abnormal findings in quarterly and PRN assessments of residents;
and the gap between the number of findings identified and the number of
referrals being made. There is a lack of accountability for follow through; a
high hand-off of responsibility between registered staff to make the referrals,
call POAS, book said appointments and initiate overall access. As a result
residents are waiting long periods for access to care or missing care
opportunities all together.
This IDEAS team focussed on the following aim; by April 2019, 60% of residents
with an abnormal finding in a scheduled/PRN head-to-toe assessment will be
offered a referral to the dental services, podiatry services
or both.
