Les communautés de pratique vous intéressent?

 

Ces communautés de pratique sont dirigées par des membres de Santé Ontario et d’autres organismes de soins de santé pour collaborer sur des programmes, projets ou initiatives d’AQ particuliers. Certaines sont publiques (c’est-à-dire que tout le monde peut participer) tandis que d’autres sont privées.

The NLOT & LTC+ Learning Community is dedicated to NLOT and LTC+ teams across the Toronto region. Our mission is to provide a platform for knowledge sharing, capacity building, and best practices while addressing common challenges. The NLOT Learning Community aims to foster an environment for peer mentorship, collaboration, and continuous learning, with the goal of enhancing care quality for residents in long-term care (LTC) settings.

The NLOT Learning Community will be facilitated through quarterly virtual and/or in-person meetings, as well as this online group space that members can subscribe to where they can log in to access:
- Tools, resources, and guidance on best practices
- Program updates and newsletters
- A group forum for networking, collaborating, sharing learnings, and problem-solving

This Community of Practice (CoP) is for Long-Term Care (LTC) Falls Prevention Clinical Leads (NP, RN, RPN, PSW, Restorative Care Worker, OT, PT) working on the development and implementation of a quality improvement project to reduce the rate of falls and fall-related injuries, and improve resident safety by sharing best practices, accessing resources, and building capacity.

CoP members will gain access to:
• Resources: Tools, evidence-based strategies, subject matter experts, and training programs to enhance falls prevention efforts.
• Quality Improvement and Best Practices: Support for implementing targeted falls prevention strategies and leveraging local data to reduce fall rates and potentially avoidable ED visits related to falls in the North East region
• Collaboration and Shared Learning: Opportunities to network, exchange experiences, and share best practices in falls prevention. The initiative also engages residents, families, and caregivers through the Stay on Your Feet program.
• Capacity Building: Strengthening staff skills through education, mentorship, and peer learning, fostering a culture of safety with ongoing training and support.

The CoP will be facilitated through regular virtual touchpoints and an online group space on Quorum.

The CoP will be supported by Jennifer Osesky (Project Sponsor), Jane Mason (NE SSPDI LTC Lead), Maria Baldia-Torres (NE SSPDI Specialist), and Jon Brunetti (NE SSPDI Population Health Lead), the North East LTC Network, and Ontario Health North East System Strategy, Planning, Design and Implementation (Long-Term Care and Population Health) team.

This community of practice (CoP) is intended for clinical, quality improvement, patient safety, and wound care teams across all settings, with a focus on pressure injuries in acute care and long-term care.

The objective of this CoP is to support the implementation of the Pressure Injury quality standard (to be updated Spring 2026) in all settings, targeting acute care and long-term care. Our group will focus on care for people who have developed or are at risk of developing a pressure injury, with a strong emphasis on early identification and prevention strategies to reduce incidence and improve outcomes.

Community members will gain access to:
- Prevention plan resources, toolkits, and guidance on best practices, including early risk assessment and intervention approaches
- Clinical and subject matter experts supporting early prevention of pressure injuries through evidence informed strategies and frontline insights
- Education and knowledge sharing opportunities such as learning modules, webinars, and other materials focused on early detection and proactive care
- Peer-to-peer connections within this online collaborative space to share early prevention successes and lessons learned

Community members will share implementation challenges and strategies, resources, data, and problem solve together to facilitate the uptake and implementation of the Pressure Injury quality standard, with a shared commitment to early prevention and continuous improvement.

This community is supported by clinical leadership, the Quality and Patient Safety Program and the Pressure Injury quality standard implementation team at Ontario Health.
