Les communautés de pratique vous intéressent?

 

Ces communautés de pratique sont dirigées par des membres de Santé Ontario et d’autres organismes de soins de santé pour collaborer sur des programmes, projets ou initiatives d’AQ particuliers. Certaines sont publiques (c’est-à-dire que tout le monde peut participer) tandis que d’autres sont privées.

The Community Stroke Rehabilitation Community of Practice (CoP) is for Regional Stroke Network staff, Ontario Health regional staff, and clinical and administrative staff involved in implementing the new Model of Care for Community Stroke Rehabilitation across Ontario. Through this CoP members will increase their awareness of leading practices and gain access to resources, tools and strategies that can be used to improve the quality of care provided within existing CSR programs and in standing up new programs. Members will work together to identify solutions to common challenges, share lessons learned, and collaborate towards future sustainability of programs.

This Community of Practice (CoP) brings together data and project leads involved in the development and implementation of the Community Stroke Rehab (CSR) Data Reporting Initiative. Our shared purpose is to strengthen data collection and reporting practices across Ontario, driving quality improvement and enhancing care for people with stroke.

The CoP will be supported through regular virtual meetings and this online collaboration space.

As a CoP member, you will have access to:
1. Virtual meetings and ongoing communication to support implementation
2. Tools, resources, and materials to aid in the roll-out of the CSR Data Reporting Initiative
3. Opportunities to collaborate, share best practices, and problem-solve with peers
4. A network for mutual support, knowledge exchange, and capacity-building

This community is a space to learn from one another, share your experiences, and contribute to advancing CSR data collection both within your organization and across the province. Your participation directly supports the provincial stroke strategy and helps to demonstrate the value and impact of CSR programs.

The CoP is facilitated by Geoff Law at Ontario Health and supported by the broader Ontario Health and Rehab Care Alliance (RCA) team.
