Les communautés de pratique vous intéressent?

 

Ces communautés de pratique sont dirigées par des membres de Santé Ontario et d’autres organismes de soins de santé pour collaborer sur des programmes, projets ou initiatives d’AQ particuliers. Certaines sont publiques (c’est-à-dire que tout le monde peut participer) tandis que d’autres sont privées.

The Central Region Palliative Care Collaborative Knowledge Centre is dedicated to supporting the implementation of Adult Community Model of Care through a cooperative and comprehensive platform. This Knowledge Centre brings together Clinical Coaches, OHT Leaders/Project Leads, Clinical Co-Leads and members of the Regional Implementation Team. This community will be facilitated through regular virtual meetings and this online group space. The community will be able to network, contribute their learnings and problem-solve together. The objectives of this collaborative are: 

- Regional Information Sharing 
- Knowledge Transfer and Bi-Directional Learning 
- Cross Community Collaboration 
- Models of Care Implementation 
- Best Practice Development 

The Knowledge Centre will be facilitated by the Central Regional Palliative Care Implementation Team at Ontario Health. We are dedicated to supporting you through proactive engagement and responsive assistance. Please reach out to your OHTs assigned implementation Lead for assistance.

The OH North East Clinical Coaching Community of Practice (CoP) is dedicated to supporting the alignment and advancement of the Model of Care Implementation for Adults in Community Settings. This online hub brings together Clinical Coaches, Clinical Co-Leads, and members of the OH North East Regional Implementation Team to collaborate, network, share resources, and problem-solve together.

The objectives of this collaborative platform are:
- Regional Information Sharing
- Knowledge Transfer and Bi-Directional Learning
- Cross Community Collaboration to support the Adult Model of Care Implementation in Community Settings
- Development & Sharing of Evidence-based Practices and materials and Quality Improvement Initiatives

The Community of Practice will be facilitated by the North East Regional Palliative Care Implementation Team at Ontario Health through regular virtual meetings and this online space. We are dedicated to supporting you through proactive engagement and responsive assistance.

The OH Toronto Palliative Care Collaborative is dedicated to supporting the implementation of the Adult Community Model of Care through a cooperative and resource-rich platform. This online hub brings together Clinical Coaches, Clinical Coach Leadership, Clinical Co-Leads, and members of the OH Toronto Palliative Care Implementation Team to network, contribute their learnings, and problem-solve together. The mission is to enable regional information sharing, bi-directional learning, and cross-community collaboration to support the Adults in Community Model of Care Implementation, while supporting the development and sharing of best practices.

This online hub will be supported by the OH Toronto Palliative Care Implementation Team. We are dedicated to supporting this group through resource sharing and proactive engagement. The mission of the Provincial Palliative Care Knowledge Exchange is to support Clinical Coaches, Regional Implementation Teams & Clinical Co-Leads implementing the Adult Community Model of Care. The Exchange is a collaborative sharing and storage platform providing easy access to tools, resources, and advice. We aim to share the following:

• A calendar of knowledge sharing events.
• Learning session recordings (via YouTube channel).
• A repository of implementation resources and tools.
• Collaborative community of practice connections for peer support and advice.

This group is facilitated by the Ontario Health Provincial Palliative Care Program Team. We are dedicated to supporting this group through proactive engagement and responsive assistance. Reach out to us at ProvincialPalliativeCareProgram@ontariohealth.ca with any questions you may have.
