Les communautés de pratique vous intéressent?

 

Ces communautés de pratique sont dirigées par des membres de Santé Ontario et d’autres organismes de soins de santé pour collaborer sur des programmes, projets ou initiatives d’AQ particuliers. Certaines sont publiques (c’est-à-dire que tout le monde peut participer) tandis que d’autres sont privées.

The DASH community of practice (CoP) is for clinical teams who are working towards a common goal of identifying, preventing and managing hospital-acquired delirium. The goal of this CoP is to strengthen the ability of hospitals across Ontario to reduce delirium rates by learning from the evidence and peer-sharing strategies.

CoP members will gain access to:

DASH is committed to the prevention and management of hospital acquired delirium, and we invite all CoP members to share resources, ask and answer questions, and network with colleagues across the province.

The Ontario General Medicine Quality Improvement Network (GeMQIN) Community of Practice brings together general medicine teams from across the province dedicated to quality improvement and better health outcomes for patients.

With a focus on using data and sharing best practices in general medicine, our members will exchange knowledge and innovate by connecting with other members.

Our community is dedicated to quality improvement in surgery for better patient outcomes. Focusing on data, and the sharing of best practice in surgery, our members will exchange knowledge, and innovate by connecting with other members.

This Ontario Vascular Community of Practice (CoP) is for vascular surgery and vascular interventional radiology teams from Ontario hospitals with designated vascular programs to work together to advance best-practice care for vascular patients in Ontario.

This CoP offers members an online space for:
• Interprofessional collaboration
• Knowledge sharing
• Collective problem-solving
• Sharing vascular program and provincial resources and updates

This CoP will be facilitated through this online group space and regular virtual touchpoints.

This community of practice (CoP) is intended for clinical, quality improvement, patient safety, and risk management teams across all health sectors to share and learn about topics related to patient safety. This CoP will give members the opportunity to come together with peers to discuss improvement opportunities and share learnings from patient safety related incidents, hospital never events, quality improvement plans, and other work to help improve the culture around patient safety.

The goals of this CoP are to:
- Collaborate, share, and learn about best practices in patient safety;
- Highlight work being done within your team or organization;
- Access quality improvement tools and resources, including those related to the Never Events Hospital Reporting initiative; and
- Engage with peers in webinars, discussions, and Q&A.

This CoP is supported by clinical leadership and quality improvement specialists at Ontario Health.
