The Primary
Care Quality Improvement Hub was established on Quorum in 2022 as a
comprehensive space to support primary care practitioners in their quality
improvement initiatives. To learn more, we sat down with Aiman Hameedi, Senior
Specialist, Quality Improvement at Ontario Health, and one of the leads
supporting the Hub.
What value
can members gain from joining the Primary Care Quality Improvement Hub?
The Primary
Care Quality Improvement Hub provides an easy-to-access library of resources
that primary care practitioners can use to support quality improvement
initiatives. The Hub also promotes upcoming events and new resources offered by
Ontario Health and other provincial and national organizations.
Members who
join the Hub are encouraged to share questions, relevant resources, best
practices and standards for all to benefit from. The Hub is meant to provide an
opportunity to learn from your peers and make connections.
Is this group
only for primary care practitioners?
While most of
our resources are primary care specific, health service providers from all
other sectors are welcome to join as most of the quality improvement supports
are applicable to the general healthcare system.
What types of
resources are available?
The Hub aims
to provide quality improvement resources suitable for all practice types, skill
level and stages of the quality improvement journey. These include internal
Ontario Health program resources as well as program resources and supports
available through other provincial and national organizations.
Resources are
found in the Attachments tab in our Hub. For example, some of our most accessed
resources include:
- Webinar
recordings from past sessions
- Monthly
Quality newsletters highlighting Ontario Health’s quality program updates
- Learning
modules: A CME accredited learning series focusing on QI in primary care
- Tools to
support improvements in Health Equity and Digital Health
We always
encourage members to share feedback on the type of resources they want to see
on the Hub through this
survey,
so we can create a more comprehensive library.
How
frequently are the resources reviewed or updated?
Some
resources, like the Quality Improvement Plan folder are updated annually as new
information is released while others are periodically reviewed and updated, as
required.
Are events
like webinars regularly scheduled?
We host
regular webinar sessions on quality improvement topics in primary care. Such
webinar announcements and session recordings are posted in the Hub and are
available under the Continuing Education folder of the Attachments tab.
For example,
the latest event we hosted was focused on the fundamentals of Quality
Improvement, emphasizing their application in primary care. Additionally, we
showcased how resources from OH and other organizations can be utilized to
enhance care for patients with type 2 diabetes.
