Ontario Health has launched a Hip Fracture Care Implementation Toolkit as part of its 3-year strategy to improve hip fracture care across the province. Developed by an advisory group of Ontario clinicians with expertise in hip fracture care, the toolkit offers practical strategies and useful resources to support hospitals in implementing the quality standard Hip Fracture: Care for People with Fragility Fractures.
Ontario Health invites hospitals to review the toolkit and select strategies relevant to their context to improve five priority areas: surgery within 48 hours, weight-bearing as tolerated, daily mobilization, screening for and managing delirium, and osteoporosis management. These priorities were selected for their potential to improve patient outcomes such as morbidity and mortality, and key hospital performance indicators including length of stay, alternate level of care days, and readmissions.
To learn more, please watch the recording of the December 11, 2025 Project ECHO Orthogeriatrics Community of Practice session, during which Ontario Health introduced the Hip Fracture Care Implementation Toolkit, walked through change strategies, and shared examples of key resources hospitals can use to drive improvement.
