professional working in Ontario’s long-term care sector? Are you actively engaged
in quality improvement (QI) initiatives related to palliative care within your
long-term care home? This CoP is for you!
Ontario’s Centre for Learning, Research and
Innovation (CLRI) in partnership with Pallium has launched a new Long-Term Care
Quality Improvement Community of Practice (CoP) focused on supporting
interdisciplinary healthcare professionals in successfully planning and
implementing palliative care-related projects under their long-term care home’s
provincially mandated Quality Improvement Plans (QIPs).
This CoP is ideal for individuals who are
action-oriented, have a foundational knowledge of QI methodologies, and have a
basic understanding of the principles of a palliative care approach.
Through this CoP, you will have the
opportunity to:
- Share and Learn: Exchange lessons learned
from previous QI projects and receive valuable feedback from your peers.
- Gain Expert Insight: Benefit from advice
and guidance provided by both content and context experts in palliative care
and quality improvement.
- Build Shared Resources: Collaboratively
develop tools and resources to support and enhance palliative care QI
initiatives.
With a focus on equity and practice-level case-based discussions, CoP members
will work together to strengthen the foundational practices needed to support
high-quality, inclusive palliative care for all individuals.
