The OHT Shared Space is for those actively participating in the development of an Ontario Health Team. Once registered, you will be able to participate one or more groups. All fields marked with an asterisk are required.

Ontario Health (OH) is collecting your personal information pursuant to section 39(2) of the Freedom of Information and Protection of Privacy Act (FIPPA) for the purpose of registering you on the OHT Shared Space.
