Our goal was to improve patients' transition from in-patient mental health to primary care. We selected Standard of Care 13: Transitions in Care from the Major Depression Quality Standard from Health Quality Ontario as our project focus, with the primary task of creating a communication process between discharge planning, family physicians and FHT staff where appropriate.
We developed processes to ensure that all care partners have the information needed to support the patient in a timely manner.We limited the scope of the study to patients being discharged following a diagnosis of Major Depressive Disorder.
