Residents may suffer and have deterioration when they are transferred to the hospital with palliative care needs that can be taken care of in the Home. This impacts both the residents and their families. It may result in increased care needs for the resident and increased support for the families.
Decreasing emergency department (ED) visits would mean that residents stay where they are comfortable and reduces the risk of complications and receiving unwanted treatment.
This IDEAS team focused on the following aim; By October 1, 2018, the transfers
to Hospital ED of Holland Christian Homes (HCH) residents will be reduced by 25% from baseline.
