The Integrated Hospice Palliative Care (HPC) program of Grey Bruce has identified the use of a quality improvement approach to ensure an integrated equitable HPC system where clients have access to coordinated and effective HPC services within their setting of choice. Grey Bruce is implementing a Palliative Care Consult and Support Team (PCCST) that would provide 24 / 7 consultation, support and mentorship services with the focus on establishing strong partnerships and creating care connections with primary care, specialized palliative care, acute care and community care. There is an opportunity to strengthen primary care delivery through access to secondary and tertiary level expertise and provide proactive, timely and seamless care. Identification and consistent implementation of best practice guidelines and screening tools such as Palliative Performance Scale (PPS) and Edmonton Symptom Assessment System Revised (ESAS-r) will be tested as approaches to ensuring patients / clients and families receive the right services at the right time and in the right place.
