There is no standard approach for reconciling medication within our electronic medical records (EMR) system.
This IDEAS team focused on the following; -By
April 30th 2019, we will ensure accurate medication information within the EMR
for 70% of the patients with visits in January - March 2019 diagnosed with
diabetes (DM) and/or hypertension (HTN) for one family physician within the
Haldimand Family Health Team (HFHT). Ensure
that patients know and understand medications that they are prescribed. Identify and understand challenges experienced by providers in trying to reconcile patient medications in the EMR.
