Our overall goal for this Rural Hastings Health Link Working Group is to improve patient and caregiver experiences so that patients with life limiting illnesses may be able to remain in their own homes with adequate supports for end of life. Specifically for the IDEAS project, we focused on how the new CCAC Palliative Care NP role would be integrated into the care of people at end of life.
