This IDEAS team (Woodstock General Hospital and Thames Valley FHT) had heard our community’s concerns about the lack of information shared between care providers upon discharge from hospital. We wondered if our organizations could come together to communicate more collaboratively; to use shared patient information in a meaningful way; to improve patient care and outcomes. We chose to focus on patients with Major Depressive Disorder (MDD).
Their aim was patients discharged from Woodstock General Hospital (WGH) to Thames Valley Family Health Team (TVFHT) will show improved PHQ-9 scores from the time of discharge from WGH to the time of the Primary Care Follow-Up Appointment (PCFUA), which is not to exceed 7 days.
Project Aim
100% of patients discharged from WGH Mental Health In-patient Unit to TVFHT will have discharge care plans sent to their Primary Care Physicians for follow-up.
