The DASH community of practice is for clinical teams working on the development and implementation of quality improvement measures to identify, prevent, manage and reduce hospital acquired delirium. It also brings together interprofessional teams who have an interest in reducing delirium rates at their site. <br/><br/>CoP members will gain access to: <br/>- Tools, resources, and guidance <br/>- Subject matter experts <br/>- Mentorship <br/>- Ongoing data reports <br/><br/>The CoP will be facilitated through: <br/>- This online group to connect members with one another <br/>- Regular virtual meetings <br/><br/>CoP members will share resources, data, implementation challenges and strategies, and problem-solve together to identify and reduce hospital acquired delirium. <br/><br/>The CoP is facilitated by the Clinical Quality Programs at Ontario Health. This CoP is supported by our program clinical leads (Dr. F. Razak, Dr. T. Jackson, and Dr. A. Verma), the Provincial Geriatric Leadership of Ontario (PGLO), and the Regional Geriatric Programs of Toronto (RGP). <br/><br/>Reach out to us at clinicalquality@ontariohealth.ca with any questions you may have.

The Ontario Surgical Network Pediatric CoP brings together pediatric surgical teams from all specialties and hospital types from across the province to form a community dedicated to quality improvement in surgery for better pediatric patient outcomes. Members will come together to coordinate surgical QI initiatives, share resources, coordinate meetings, and discuss challenges and successes.
