This webinar is intended to support individuals leading the
implementation of the palliative care indicator in their quality improvement
plans.
The webinar will provide a deeper dive into:
- Advanced Care
Planning, Goals of Care and Health Care Consent in Ontario
Objectives:
- Clarify the difference between health care consent, advance care planning, and goals of care
- Understand the role of a substitute decision maker and how power of attorney fits into the process
- Support awareness of available Ontario-based tools and resources

Quality Improvement Help Session: Deep Dive into Data

In this session we will focus on helping you with measurement and data.
Target Audience:
- Individuals within an organization who are leading the implementation of the early identification and assessment of palliative care needs Quality Improvement Plan (QIP) indicator
Objectives:
- Analyze the options for defining the target population
- Distinguish the difference between the indicator’s numerator and denominator
- Develop a process that ensures accurate measurement
- Explore and dissect an example from the field
