Hello, this is more of a question than a post. I am the QI lead attached to Pressure Ulcer Prevention in my organization. We currently collect data on average length of stay, the number of hospital acquired pressure ulcers as well as the rate of hospital acquired ulcers (per 1000 discharges). Right now the data we collect is based upon patient discharge. We are looking to create quarterly scorecards with this information to disperse at the unit/department level for staff to discuss at huddles and to drive improvement where required.
I am interested to see how do other Hospitals collect data on hospital acquired pressure ulcers. My main questions are:
Is your data based upon discharge?
Do you have an electronic medical record where this information is available in real time and you can pull reports?
Are all hospital acquired pressure ulcers being reported in your incident reporting system?
Do you have a target or benchmark that you use to measure performance from year to year, and if so, what algorithm did you use to come up with that target and does it change yearly?
