Since the launch of the Health Links Approach to Care by the Ministry of Health and Long-Term Care in 2012, there have been many achievements to be proud of!
Last year, the Waterloo Wellington LHIN conducted an evaluation of the Health Links Approach to Care within its subregion through a series of Benefits Realization Case Studies with support from the eHealth Centre for Excellence.

1. Cambridge North Dumfries
Achievement: The development of a coordinated referral process for complex patients to access services results in an efficient and streamlined referral process, and appropriate service coordination and care for patients.
2. Guelph
Achievement: The flexibility inherent in the Health Links approach allows providers to determine their own processes that have the ability to promote patient-centred care, resulting in greater engagement and satisfaction for Health Links patients.
3. Kitchener-Waterloo
Achievement: An interprofessional, community based team approach for complex patients allows providers to offer collaborative, holistic care while addressing patients’ connection with the wider system, resulting in more coordinated care and improved patient outcomes.
4. Rural Wellington
Achievement: Rurality is a social determinant of health that has a significant impact on residents living with complex conditions, as residing in a non-urban location impacts their ability to access appropriate care. The team outreach approach bridges the gap between patients and community resources, empowering patients to overcome their barriers.
